I’m not sure it counts as “missing”, but Romana had two different forms,
being a timelord. Both quite different, so separating them might be wise.

Personally, I don’t think Captain Jack should be counted as one of the Doctor’s companions. A cool reocurring character but not a companion.

I picked Romana, but out of the two versions of her, I’d pick the first one.
I wouldn’t really consider Brigadier Lethbridge-Stewart as a companion. At least not any more than Sergeant Benton or Captain Yates.
But I do think that Adam Mitchell (from the episodes “Dalek” and “The Long Game”) should be considered as a companion.
